An upsetting new report details the Air Force’s “gross mismanagement” of human remains from U.S. soldiers killed at war, and has resulted in the disciplining of three senior officials. News of two lost body parts “equates to an aggregate success rate slightly greater than 99.9 percent” at the mortuary, a report said, but “the success rate for families of the deceased in the two individual cases is zero percent,” according to an internal investigation, which dubbed the misplaced remains “mission failure.”
